Here at Mountainview, we use an online referral form that allows us to quickly access which students we need to meet with individually. We often meet individually with students when there has been a traumatic/emotional event, they have a problem and need some help, they need help with coping skills, or anything related to their emotional well-being.
Please use this form to refer your student to one of the counselors for an individual check-in. Only the counselors have access to the response sheet of this document; therefore, it's completely confidential!
